Prior to your first appointment, you will be required to complete a confidential health history form. This form must be updated once a year or any time there are significant changes to your health history (surgery, major illness, etc.). You are responsible for notifying me of these changes before the start of your next appointment. The information in your file can only be released with your consent or as required by law. Cancellations/Rescheduling

Life happens. However, as a solo practitioner, 100% of my income comes from you, my valued clients! When you cancel with little to no notice, this affects my livelihood. You are also taking appointment time away from someone else who could benefit from my care; therefore, the following policies are not punishment but are in place to protect my time:

If you are unable to make a scheduled appointment, you must cancel/reschedule at minimum 48 hours in advance. This can be done by logging in to the online scheduling system, or contacting me via text or email. If less than 48 hours’ notice is necessary, please do so via phone call or text. If your appointment time cannot be filled, your card on file will be charged 100% of the full session rate within 1 hour of the canceled appointment time. No exceptions.

Same-day cancelations and no-shows will be charged 100% of the full session rate, no exceptions. Three or more cancellations with 48 hours or less notice within a 6-month period may result in dismissal as a client. Any no-show without response to my communications will be an automatic dismissal and will be notified as such via email.

Because my services are in demand, continuous rescheduling of a single appointment more than twice will result in that appointment being removed from the calendar. Pre-payment will then be required for all future appointments. If the pattern is continued with a pre-paid future appointment, the result will be forfeiture of pre-payment and dismissal as a client. Example: scheduling 2 months ahead and within one week rescheduling out another month, and within one week again rescheduling it out another month.

Appointment reminders are automated and sent via email 5 days prior and, if unconfirmed, again via text 3 days prior to your scheduled appointment time.

Arriving on Time

You must arrive at least 5-10 minutes early for your appointment to allow adequate time for intake & conversation. Your appointment time is the start of your hands-on time. If you arrive exactly on time or late, I can give you only the remainder of your scheduled time. PLEASE NOTE: Arriving more than 10 minutes early for an appointment does not guarantee an early start to your session. If you need to begin early for any reason, please let me know in advance and I will do my best to accommodate.

Refusal of Service & Zero Tolerance

For my safety, I have the right to refuse/discontinue treatment at any time. Reasons may include, but are not limited to illness, infectious/contagious/unidentified skin conditions, etc... Should your service be refused or discontinued, it is considered a same-day cancelation and you remain responsible for the full session rate. Rescheduling of the session will be determined at the time of refusal/discontinuation.

Suggestive and/or inappropriate or offensive comments or behavior will not be tolerated and will result in an immediate end to the session and automatic dismissal. The full session rate will be due.

Refund Policy

All retail products, prepaid services, gift cards, packages, and the like are non-refundable. If you are unable or unwilling to use any of the aforementioned items that have already been purchased, it is recommended that they be gifted to another who may find them of value. If you wish to “return” an unwanted purchase vs. gifting, no monies will be refunded and the “returned” item(s) will be donated or credited to a charity of my choosing.

Gift Card Policy

The purchase value of gift cards will always remain valid. In the event of a rate increase [where the previously purchased gift card has not yet been redeemed], the difference beyond the value will be owed at time of service. Gift cards MUST be presented at payment to be redeemed. Gift cards not present or e-gift cards missing important numbers, barcodes, etc.... will not be accepted and full payment will remain due by the client who held the appointment.
